Fees and Payments

Evaluation Fees

The fees for evaluation services differ depending on the specifics of the evaluation needed and are in line with other neuropsychological service providers in the area. If you have any questions about the specific fees, please do not hesitate to contact me to discuss.

Consultation Fees

Consultation fees are billed on an hourly basis and vary depending on the length of the desired consultation.

Insurance

I do not accept insurance. However, while I am not on insurance panels, I will provide you with a detailed bill that you can submit to your insurance for out of network reimbursement. Please check with your insurance to determine if you have out-of-network benefits.

Forms of Payment

I accept cash, personal checks, and payments through Zelle. Credit card payments are also accepted with additional processing fees.

Right to Receive a Good Faith Estimate of Expected Charges

Under the No Surprises Act

Under the new regulations implemented as part of the “No Surprises Act,” health care providers must give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.

You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit https://www.cms.gov/nosurprises  or call 1-800-985-3059

In Person and Telehealth Services

Face to Face Testing

Neuropsychological assessments are conducted mostly in my office face to face with the patient whenever possible. To ensure your safety during the coronavirus outbreak, CDC recommendations for cleaning and personal protection are closely followed.

Telehealth Services

Consultations and clinical interviews are conducted primarily online through a secure, HIPAA-compliant platform. You will be emailed a personal link for each telehealth meeting prior to your appointment.

Scheduling

Initial  appointments are scheduled after a brief consultation with Dr. Ostroy to ensure that the services offered are clinically appropriate for your needs. You will be sent an intake packet that would need to be completed and submitted before the initial appointment can be scheduled.

Cancelations

A deposit is required at the time of scheduling your evaluation or consultation appointment. The deposit is refundable up to 48 hours prior to the appointment. Because the time is reserved for you and cannot be filled on a short notice, a full fee is charged after that. Exceptions may be made in case of emergency and serious illness.
